Once you've started your Personal Independence Payment (PIP) claim by phone, you’ll be sent a form called 'How your disability affects you' — commonly referred to as the PIP2 form.

This form is your opportunity to explain clearly how your condition impacts your daily life and mobility. Here’s how to complete it properly and give yourself the best chance of success.

🖊️ Fill in Every Section

The PIP2 form includes sections about daily living activities (like cooking and washing) and mobility (like moving around and planning journeys). Answer every question, even if you feel it doesn’t fully apply.

Use real-life examples. Explain what happens on both good and bad days. Be honest and specific.

📌 Use Extra Sheets if Needed

If you run out of space, you can attach extra paper. Write your name and National Insurance number at the top of each additional sheet, and clearly indicate which question it relates to.

🧠 Be Clear About Your Support Needs

Don't assume the DWP will ‘read between the lines’. If you need help with tasks like dressing, remembering appointments, or preparing food, explain how and why.

📂 Gather and Include Supporting Evidence

You can include medical evidence such as:

  • Letters from doctors or consultants
  • Prescription lists
  • Physiotherapy or mental health reports
  • Statements from carers or family members

Send copies, not originals, and include them with your completed form.

📮 Send It Back on Time

You’ll usually have one month from the date the form was sent to return it. The return address will be on the form, and you should post it using a tracked or recorded delivery service if possible.

You can get help from a local welfare rights advisor, Citizens Advice, or disability charities like Scope or Mencap. They can help you understand the questions and word your answers clearly.
